Purpose: The purpose of this webinar is to assist social workers to acquire the knowledge and skills to identify and address difficult decisions and manage conversations in end-of-life care.

Objectives:

  1. Identify barriers and implement strategies to addressing difficult topics in end-of-life care 
  2. Develop a working knowledge of advance directives and language to discuss these with families and patients
  3. Utilize the surrogacy hierarchy as set out in the Family Healthcare Decisions Act

Why Do I Need to Know This?
Patient-centered care is based upon decision-making and advance care planning commensurate with the values and wishes of the patient and family. Yet social workers are not well-trained in advance care planning and may therefore avoid difficult conversations at the end of life. A working knowledge of advance directives and exposure to best practices will enable the social worker to participate more fully in end-of-life communication and decision-making. 

Target Audience:  Social workers and healthcare coaches

Criteria for Successful Completion:

  1. Attendance at entire session
  2. Submission of completed evaluation form
  3. Successful completion of a posttest (80% passing grade)
