Purpose: The purpose of this webinar is to describe the importance of the family meeting as a best practice for advance care planning and caregiver support in hospice and palliative care.

Objectives:  

1. Discuss best practices related to the family meeting in hospice and palliative care
2. Describe core communication skills integral to the family meeting
3. Discuss challenging clinical scenarios in the context of the family meetings

Target Audience: Primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, allied health professionals, fellows, residents, creative arts therapists, and medical students

Criteria for Successful Completion:

1. Attendance at entire session
2. Submission of completed evaluation form
3. Successful completion of a posttest; 80% passing grade (mandatory to receive CE credit for social workers)

Physicians: The AAFP has reviewed MJHS Institute for Innovation in Palliative Care Interprofessional Webinar Series - On Demand (3), and deemed it acceptable for AAFP credit. Term of approval is from 06/27/2024 to 06/26/2025. Physicians should claim only the credit commensurate with the extent of their participation in the activity. This session The Family Meeting in Palliative Care: Best Practices is approved for 1.0 enduring AAFP Elective credit.

Case Managers: This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers. The course is approved for 1.0 CE contact hour.
