Purpose: The purpose of this webinar is to help social workers acquire the knowledge and skills to define different levels of care in palliative care and hospice and identify the role of the social worker.

- Define levels of care in palliative care and hospice
- Identify trends in care at the end of life
- Recognize criteria for admission to hospice
Why Do I Need to Know This?
Palliative care and hospice are beneficial for patients and caregivers facing advanced and terminal illness, but they are poorly understood, and there are misperceptions which lead to late referral within a few days of death. Every team member has an important role to play.
Target Audience: Social workers and healthcare coaches
Criteria for Successful Completion:
- Attendance at entire session
- Submission of completed evaluation form
- Successful completion of a posttest (80% passing grade)
