Purpose: The goal of the webinar is to help you develop skills to determine eligibility and to encourage referral of patients for palliative care and hospice.

Objectives:

  1. Review the diagnosis and disease processes within a case
  2. Determine hospice eligibility
  3. Encourage referral of patients for palliative care and hospice

Target Audience:  Social workers and healthcare coaches

Criteria for Successful Completion:

  1. Attendance at entire session
  2. Submission of completed evaluation form
  3. Successful completion of a posttest (80% passing grade)
