Purpose: The goal of the webinar is to shine a light on the importance of care transitions in medical care and highlight best practices to enhance continuity of care.
Objectives:
- Identify the role of the social worker in the hospital, home care, ambulatory and long term care settings
- Use best practices to complete a patient- and caregiver-centered care plan
- Use best practices for appropriate biopsychosocial assessment and intervention focused on coordination of services and continuity of care
Target Audience: social workers and healthcare coaches
Criteria for Successful Completion:
- Attendance at entire session
- Submission of completed evaluation form
- Successful completion of a posttest (80% passing grade)
